The paper deals with the choice of an anesthetic mode and postoperative analgesia for patients with large ventral hernias and details the present views and pathogenetic aspects of the mechanisms of the pain syndrome occurring in this cohort of patients. The conducted analysis of both groups of patients under endothracheal and combined anesthesias (a combination of endotracheal anesthesia and peridural analgesia) showed the efficiency of a choice for combined anesthesia in preventing the development of respiratory failure in patients with ventral hernias. В статье представлена проблема выбора анестезиологического пособия и послеоперационного обезболивания больных с большими вентральными грыжами, подробно изложены современные представления и патогенетические аспекты механизма синдрома боли, имеющего место у данной категории пациентов. Проведенный анализ двух групп больных с использованием эндотрахеального наркоза и комбинированной анестезии (сочетание эндотрахеального наркоза и пери-дуральной анальгезии) показал эффективность выбора комбинированной анестезии с целью профилактики развития дыхательной недостаточности у больных с вентральными грыжами.
