Purpose: Development and an assessment of efficiency of a technique of phakic intraocular lenses (FIOL) at cataract phacoemul- sification.Methods: 25 FIOL, a pupillary lens RSC-3, a posterior chamber lens, production of MNtK, Russia, and posterior chamber lens ICL S4 StAAR were removed. Before complex surgical intervention (FIOL + phacoemulsification) were carried out standard ophthalmo- logic examination, and also automatic detection of density of endothelial cells.Results: the developed technique of removal of FIOL included subconjunctival anesthesia, formation of a corneal tunnel, filling of the anterior chamber viscoelastic, FIOL mobilization, removal of the bottom haptic element in the anterior chamber, removal of the top haptic element in the of anterior chamber, FIOL removal. In the early postoperative period in 8% transitory hypertensia was observed, in one case (4%) cornea hypostasis developed. In 92.4% of cases of the maximal visual acuity with correction was corresponded to the planned. Loss of endothelial cells after operation made 4.8% that in full corresponds to standard loss of cells after not complicated phacoemulsification.Conclusion: the technique of removal of FIOL developed as part of this research allows enough effectively and to carry out safelyto patients surgical treatment of the complicated cataract with a phakic intraocular lenses explantation.
Цель. Разработка и оценка эффективности методики удаления факичных интраокулярных линз (ФИОЛ) при факоэмуль- сификации катаракты (ФЭК).Методы. Выполнено 25 удалений (9,7%) ФИОЛ из 258 имплантированных в разные сроки. Заднекамерная ФИОЛ отече- ственного производства была удалена в 3,7% случаев в связи с развитием катаракты в течение 4-5 лет. Удаление заднекамер- ной ФИОЛ ICLS4 StAAR проведено в 1,5% случаев в достаточно ранние (1-2 года) сроки. Удаление зрачковой линзы «RSC-3» было произведено в 4,5% случаев в сроки от 6-7 лет. До и после комплексного хирургического вмешательства (удаление ФИОЛ + ФЭК) выполнялось стандартное офтальмологическое обследование, а также автоматическое определение плотно- сти эндотелиальных клеток.Результаты. Разработанная техника удаления ФИОЛ включала субконъюнктивальную анестезию, формирование ро- говичного тоннеля, заполнение передней камеры вискоэластиком, мобилизацию ФИОЛ, выведение нижнего гаптического элемента в переднюю камеру, выведение верхнего гаптического элемента в переднюю камеру, удаление ФИОЛ. В раннем послеоперационном периоде в 8% случаев наблюдалась транзиторная гипертензия, в одном случае (4%) развился отек рого- вицы. В 92,4% случаев величина максимально корригируемой остроты зрения вдаль соответствовала планируемой. Потеря эндотелиальных клеток после операции составила 4,8%, что в полном объеме соответствует стандартной потере клеток по- сле неосложненной ФЭК.Заключение. Разработанная в рамках настоящего исследования методика удаления ФИОЛ позволяет достаточно эффективно и безопасно выполнять хирургическое лечение осложненной катаракты с одновременной эксплантацией факичных линз. |
