Purpose: to determine the long-term efficiency and safety of implantation Ex-PRESS open-angle glaucoma device to patients at advanced stage of POAG.Methods: Seven male patients underwent penetrating filtering surgery with implantation of the Ex-PRESS shunt over the lack of stabilization of visual functions on hypotensive drugs.Results: Average monitoring of patients was 19.96±9.73 months. IOP in postoperative period was 16.14±3.67 mm Hg (by Makla- kov). the reduction of IOP was 12.72 mm Hg (40.2%). Cell density didn’t differ from double (2879±87 cell/mm2 и 2798±67 cell/mm2, p>0.05).Conclusion: Long-term gradual reduction of IOP and safety of implantation Ex-PRESS glaucoma device may be a reason of choicethis type of surgery for patients at advanced stage.
Цель. Определение эффективности и безопасности применения дренирующего устройства Ex-PRESS у пациентов с про- двинутыми стадиями первичной открытоугольной глаукомы в отдаленные сроки наблюдения.Методы. В исследование были включены 7 пациентов, которым были выполнены оперативные вмешательства по поводу глаукомы с использованием дренирующего устройства Ex-PRESS в связи с отсутствием стабилизации зрительных функций на фоне применяемой гипотензивной медикаментозной терапии.Результаты. Средняя длительность наблюдения за пациентами в послеоперационном периоде составила 19,96±9,73 месяцев. Уровень ВГД составил 16,14±3,67мм рт.ст., а его среднее понижение по отношению к базовому — 12,72мм рт.ст. (40,2%). Плотность эндотелиальных клеток роговицы в оперированном глазу не отличалась от таковой в парном (2879±87 кл/мм2 и 2798±67 кл/мм2, p>0,05).Заключение. Длительная эффективность и безопасность понижения офтальмотонуса при имплантации дренирующего устройства Ex-PRESS могут служить основными аргументами в выборе хирургического лечения пациентов с продвинутыми стадиями болезни в случаях, когда необходимо особенно тщательно оценивать риск интра- и послеоперационных осложне- ний. |
